1. Consent to Use of Transportation

1. I hereby give consent for my child to be transported to and from the Event and to participate in any transportation arrangements made by GLD for purposes of facilitating any Event related activities. I further consent to such transportation being provided by private motor vehicles driven by adult volunteers, church leaders, parents and/or appointed drivers acting on behalf of GLD.
2. I acknowledge that transportation will be provided by private motor vehicles and not by public transport, and that such vehicles will, to the best of GLD’s knowledge, be roadworthy and suitable for the transportation of children. I voluntarily consent to my child being transported in such vehicles.

2. Transportation Indemnity

1. I acknowledge and agree that the transportation of my child to and from the Event, and to any activities related thereto, is undertaken entirely at my own risk, and I accept all inherent risks associated with travel in private motor vehicles.
2. I hereby indemnify and absolve GLD, its pastors, leaders, organisers, volunteers, drivers, assistants, and/or any person acting on behalf of GLD from any liability, damages, injuries, losses, costs, or expenses of whatsoever nature arising from or in connection with:
   1. The transportation of my child to and from the Event;
   2. The transportation of my child to and from the Heron Bridge for the purpose of participating in activities related to the Event;
   3. Any motor vehicle accident or incident in which my child may be involved during the course of the Event;
   4. Any injury that my child may sustain during transportation or participation in the course of the Event related activities;
   5. Any loss of, or damage to, property that may occur during transportation or participation in the course of Event related activities;
   6. Any delay, breakdown, or other incident occurring during transportation or participation; and
   7. Any act or omission by the driver, or by any other person involved in the transportation of my child.
3. I further agree that I shall not institute any legal proceedings or bring any claims of any nature against GLD, its leaders, volunteers, drivers and/or any person acting on behalf of GLD in respect of any incident, accident, or event that may occur during the transportation of my child or his/her participation in Event activities.

3. General Indemnity

1. I hereby indemnify and absolve GLD, its pastors, leaders, organisers, volunteers, and/or assistants from any claims, damages, injuries, loss, or liability of whatsoever nature arising from my child’s attendance or participation in the Event and all Event related activities.
2. I acknowledge that my child’s participation in the Event is voluntary and entirely at my own risk. I understand that GLD’s leaders, pastors, volunteers, and/or any assistants will take all reasonable care to ensure the safety of my child, but such measures do not eliminate the risk of injury, loss, or damage, and GLD and its representatives cannot be held liable for any such occurrence.

4. Medical Consent

1. In the event of an emergency, I hereby authorise GLD leaders or any responsible adult acting on behalf of GLD to obtain medical treatment for my child if deemed necessary, and I agree to be responsible for any costs incurred in connection with such treatment.
